Well the Hulk was pretty much on death's door because of the poison that Madman injected into Bruce Banner's body, so Madman pretty much controls the fight physically. Fortunately for Hulk Phil Sterns is pretty much nuts and transformed into his human form and spelled out the means for Hulk's victory. Hulk poisons Madman and gets the antidote when Madman goes for it.

Sensational She-Hulk #21- She-Hulk vs. Abominatrix I
Abominatrix's boss has her kidnap She-Hulk's friend Weezi. She-Hulk comes looking for her and the fight ensues. The fight ends prematurely when Abominatrix's boss, Jasper Keating, reveals that he kidnapped Weezi for information on an atomic bomb's location.

Sensational She-Hulk #23- She-Hulk vs. Abominatrix II
When She-Hulk returns to rescue Weezi Abominatrix knocks her out with a giant safe. Abominatrix and her associates flee to Las Vegas. She-Hulk is revived by Weezi and her daughter.

She-Hulk knows the location of the atomic bomb. In fact the fight winds up taking them directly there. She-Hulk tries to warn Abominatrix, but she doesn't believe her. She-Hulk winds up saving the day, but does not take down Abominatrix.

Incredible Hulk vol. 2 #373- Hulk vs. Doc Samson XV
After the Savage Hulk returned Doc Samson comes to a startling conclusion- since Banner is an MPD there is no reason the gray Hulk and savage Hulk could not co-exist. When night comes the cornered Bruce Banner turns into the gray Hulk. Hulk fakes out Samson that he caught exploding grenade shrapnel in the eyes, but truthfully Samson was never really in it. When he compares the gray to the savage Hulk it angers Hulk. One face to the ground later and Hulk leaps away with Betty.

Incredible Hulk vol. 2 #383- Hulk vs. Abomination XV
Hulk just witnessed the disappearance of Rick Jones and half the room as Thanos was in posession of the Infinity Gauntlet, but Hulk didn't know that and blamed Abomination. Just as Hulk is about to cream Abomination he is sucked away in a vortex. Abominaton considers it divine intervention in the face of armageddon.
